reducing calories too low: this causes a loss of gym performance, loss of muscle, weight loss that quickly stalls, weight rebound and burnout.
neglecting the post-diet maintenance phase: this phase allows you to recover after a diet and creates a new set-point for your new bodyweight and eliminates weight gain "rebound".
gaining too much fat while building muscle: increasing calories to build muscle is a must, but no one wants to gain more fat than absolutely necessary.

poor technique: leads to poor results and increases chances of injury.
